Who we are

Information we collect

Information you submit. When you use the consultation request form on our Contact page, we collect the information you enter: your first and last name, email address, practice or organization name, and message, plus your phone number and service of interest if you choose to provide them.

Information collected automatically. Our web server and hosting provider may automatically record technical information, such as your IP address, browser type, and the pages you request, in standard server logs. Our website’s security service, Wordfence, also records the IP address, browser type, requested address and time of activity it considers suspicious, such as failed sign-in attempts and requests its firewall blocks, including any username entered on the sign-in page.

Please do not submit patient information

Our website contact form is intended for business inquiries only. Please do not include patient names, medical records, or any other protected health information (PHI) in form submissions or in emails sent through this website.

How we use information

• To respond to your inquiry and schedule a consultation.
• To communicate with you about the services you asked about.
• To operate, maintain, and protect the security of this website, including detecting and blocking malicious traffic and repeated failed sign-in attempts.

We do not sell your personal information.

How information is stored and shared

Form submissions are saved in our website’s database and sent by email to our business inbox. Our website is hosted by a third-party hosting provider that stores website data on our behalf. Our website security service may share the IP addresses involved in suspicious activity with its provider, as described under Third-party services. We may also disclose information when required by law or to protect our rights.

Cookies

This website does not currently use analytics or advertising cookies. The contact form’s verification feature uses a temporary session cookie to confirm that a person, rather than an automated program, is submitting the form. Additional cookies, including cookies from our website security service, are used only on the website’s administrator sign-in page and when a site administrator is signed in to manage the website.

Third-party services

This website loads fonts from Google Fonts. When a page loads, your browser connects to Google’s servers, which receive your IP address and basic browser information. Google’s use of this information is described in the Google Privacy Policy.

This website uses Wordfence, a security service from Defiant, Inc. in the United States, to protect against attacks. When the site detects suspicious activity, such as repeated failed sign-in attempts, it may share the IP address involved with Wordfence’s security network. How long we keep information

We delete website inquiry entries after 12 months. We delete related inquiry emails after 24 months unless we need them for an ongoing client relationship or a legal retention requirement.

Our security service’s traffic log keeps entries for no more than 30 days.

Your choices

You may ask us to access, correct, or delete the personal information you submitted through this website by contacting us at customerservice@bandemgmt.com. We will review and respond to your request.

Children’s privacy

This website is intended for businesses and professionals. It is not directed to children under 13, and we do not knowingly collect personal information from children.

Security

We use reasonable measures, including encrypted (HTTPS) connections, a website firewall, and sign-in protection, to protect information submitted through this website. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
